Waiver and Release Liability

I/We hereby understand and acknowledge that the training, programs, classes and events held by Court 16 Inc. and its affiliate companies may expose my child or me to many inherent risks, including accidents, injury, illness or even death. I/We assume all risk of injuries for me or my child associated with participation in Court 16 Inc. and its affiliate companies activities including, but not limited to, falls, contacts with other participants, equipment or tennis balls, and all other such risks being known and appreciated by me.

I/We accept full responsibility for the use of any equipment, facility privilege, or service whatsoever, owned and operated by this club at my/our own risk and shall not hold this Court 16 Inc., and its affiliate companies, its shareholders, officers, employees, representatives, agents and property owner of event premises liable for any loss, claim, injury or liability sustained or incurred during voluntary participation at Court 16 Inc. and its affiliate companies and their programs.

I/We hereby acknowledge my responsibility in communicating any physical and psychological concerns that might conflict with my participation or the participation of my child in any activity. I/We acknowledge that I/We or my child are physically fit and mentally capable of performing the physical activity I/We choose to participate in.

After having read this waiver and knowing these facts, and in consideration of the acceptance of my participation and/or the participation of my child in the services provided to me/my child by Court 16 Inc. and its affiliate companies, I agree, for myself, and anyone acting on my behalf to HOLD HARMLESS, WAIVE, and RELEASE Court 16 Inc., and its affiliate companies, its officers, agents, employees, organizers, representatives, successors and property owner of event premises from any responsibility, liabilities, demands of every kind, losses, injuries, property damage or claims or any kind arising out of my participation and/or the participation of my child in Court 16 Inc. and its affiliate companies activities or events; or arising out of use or intended use of any facilities, equipment or other property of Court 16 Inc., and its affiliate companies; and I/We covenant not to sue, therefore.

I hereby attest and verify that I have full knowledge of the risks involved in Court 16 Inc.and its affiliate companies activities, that I assume those risks, and that I will assume and pay my own medical expenses and emergency expenses in the event of accident, illness or other incapacity, regardless of whether I have authorized such expense.

I further agree to provide appropriate medical information to Court 16 Inc.and its affiliate companies if I am required to by Court 16 Inc. and its affiliate companies. In addition, in case of accident or injury to me and/or my child and if an emergency contact person cannot be reached, I grant Court 16 Inc. and its affiliate companies permission to obtain medical attention if necessary, for which I will be financially responsible.

I understand and agree that Court 16 Inc. and its affiliate companies are not responsible for property that is lost, stolen, or damaged while in, on, or about the premises.

Media Release

We reserve the right to reasonably utilize photos or videos taken at Court 16 Inc. and its affiliate companies that may include you as promotional material for social media or others. 

I/We indicate that I/We have read and understood this Media Release. I know this is a release of liability, and I voluntarily agree to its terms.
